
A new study in *The Lancet* reveals that 60% of liver cancer cases could be prevented by addressing key risk factors: hepatitis B and C, alcohol-related liver disease, and metabolic conditions like obesity. Without intervention, annual global cases could nearly double to 1.5 million by 2050.

Liver cancer, the sixth most common cancer, claims over 700,000 lives yearly. While viral hepatitis remains a leading cause, alcohol use and metabolic dysfunction—linked to obesity and diabetes—are growing threats, projected to account for a third of new cases by 2050.
Most liver cancers develop in people with cirrhosis, irreversible scarring caused by chronic inflammation. Hepatitis B and C, excessive alcohol consumption, and metabolic dysfunction-associated steatotic liver disease (MASLD) all contribute to this damage. MASLD, affecting 40% of adults globally, often progresses silently to cirrhosis and cancer.

Experts emphasize prevention through vaccination, lifestyle changes, and early screening. Dr. Brian P. Lee (USC) notes liver cancer’s tragic preventability, while Dr. Ahmed Kaseb (MD Anderson) warns that alcohol and MASLD risks are often overlooked. Current guidelines recommend monitoring for cirrhosis, but many MASLD patients go undetected until advanced stages. Dr. Mary Rinella (University of Chicago) advocates primary care screening using Fib-4 scores for high-risk individuals, including those with obesity or diabetes. MASLD can be reversed with diet, exercise, and new weight-loss drugs.

Alcohol-related liver disease is also rising, particularly among women and those with metabolic syndrome, who face compounded liver damage. Heavy drinking and obesity together significantly increase risk. With alcohol use and obesity rates climbing, experts predict a continued surge in liver disease. “The burden will remain high unless we act,” says Dr. Rinella.
